'Swing bed' transitional care

Sometimes we need extra recovery time and rehabilitation after a hospital stay or surgery. This transitional care, known as "Swing Bed" under Medicare, can be the answer if you or a loved one no longer needs to stay in the hospital but still needs rehabilitation and/or nursing services before going home.

Gundersen St. Elizabeth's Swing Bed program provides a combination of independence, customization and skilled care. Our multi-disciplinary care team is focused on helping you achieve better health in a friendly and supportive environment. 

Frequently asked questions

How do I qualify for Swing Bed?

You can recover in a Swing Bed program closest to you if:  

  • You spent at least 3 nights in a hospital within the past 30 days  
  • Your provider finds you need daily skilled care after your treatment in the hospital
  • You have Medicare A or another hospital-based insurance with skilled care benefits
  • You’re willing to participate in your care plan and make progress toward your goals
