Skilled nursing care in Wabasha

Our main campus in Wabasha is home to Gundersen St. Elizabeth's Care Center - Grant Boulevard, a skilled nursing facility with 20 beds. We provide holistic, person-centered care for residents who need short- or long-term care. Our team of professionals works together with each resident's family to design a personalized care plan that includes medical care and therapeutic support, meaningful activities, social connections and family involvement. Our comfortable surroundings and amenities contribute to their well-being. Our care center is a Veterans Affairs (VA) contracted facility.


When setbacks happen, recovery matters most. Whether you or your loved one had a planned or unplanned hospital stay, our professional team of nurses, therapists and providers are ready to help. We're committed to rehabilitating each resident to their highest level of independence and understand the priority of our residents getting home safely, swiftly and back to doing what they enjoy.

When living at home independently is no longer an option, our goal is to help make the transition to long-term care smooth and worry-free. We welcome you and your family to tour our facility and meet our team. We'll review medical history and prioritize care planning to better understand the unique needs and preferences.

Respite care can be relief and reassurance when a patient or caregiver needs it most.

Offering nursing services and no required length of stay, you can count on our team to provide quality around-the-clock care for 5 days or 5 weeks - it’s up to you.

Contact our social work team to discuss the direct admit process and options for financial coverage. Respite services are not dependent upon hospice enrollment and may be covered by Medicaid or private pay.
