The face of healthcare is changing and so are we. More and more, patients are becoming active participants in their own health via increased self-awareness, diet, exercise and preventive care. In support of this trend, we are proud to offer you free, online self-care tools.
WHAT SELF-CARE TOOLS DO WE OFFER?
Health risk assessments – We have risk assessment tools that cover a variety of common health issues. Risk assessments take about five minutes to complete and calculate your personal risk for a given health issue. We believe that being informed is powerful and essential for helping to improve the health of our community. Do your part and take one or more of our free risk assessments:
Self-care planners – Sometimes you aren't sure if you health issue is serious enough to warrant a visit to the doctor. Our free self-care planners provide an opportunity to follow a customized, guided exercise program to help you manage your health issue on your own. We offer self-care plans for:
Our self-care planners include six weeks of exercises, videos and results tracking. There are three levels so the plan can be customized to match your specific symptom profile, which is determined based on a series of questions. At the end of six weeks, you can re-evaluate your progress by completing the Self-Care Planner questionnaire. The results will let you know if you're improving, staying stable or getting worse, and suggest next steps.
