If you have questions, concerns or feedback regarding your experience, we encourage you to:

  1. Talk with your nurse or doctor, or the unit/department charge nurse, nursing supervisor or manager.
  2. Call us at (608) 775-7676 Monday through Friday during business hours to speak with a patient representative. You may leave a message for the Patient Experience team at any time.

Our Boscobel and West Union locations participate in The DAISY Award, a nationwide program that rewards excellence in nursing. If you have been cared for by a remarkable nurse in Boscobel or West Union, this is your opportunity to honor them for the difference they’ve made in your life.

Nominate a nurse

In your written correspondence, include the following details, please:

  • Name and birth date of the patient. If you are not the patient, provide your name and contact information
  • Date of service
  • Location of the facility and the name of the department you visited
  • Your comments about your experience
  • Your name, address and phone number, if you would like follow-up contact
