Gundersen Health System offers COVID-19 vaccine for all patients and non-patients ages 6 months and older.
How to schedule your COVID-19 vaccine:
- Gundersen patients can schedule their appointment through MyChart or by calling (608) 775-6829.
- Adult non-patients can schedule their appointment by calling (608) 775-6829.
COVID-19 vaccine is available during scheduled appointments at most Gundersen primary care locations. Please contact your provider’s location to check COVID vaccine availability.
It is also available at the Gundersen Onalaska Clinic, Level 4, by appointment, Monday through Friday, 8 a.m. to 4 p.m.
