At Gundersen Boscobel Area Hospital and Clinics, we don't just see symptoms, illnesses and treatments. We see you. We take the time to understand your concerns, your lifestyle and your individual health needs.
We are committed to providing high-quality care. We believe that goes beyond medical care to include respect, compassion and dignity. Our patient care is centered on you and your loved ones.
We are always looking for ways to improve your patient experience. Please contact us at (608) 375-4112 with any feedback about your care.
Advance care planning
Advance care planning is the process of coming to understand, reflect on, discuss and plan for a time when you cannot make your own medical decisions. The process is crucial to assist those closest to you and your physician(s) in respecting your choices regarding the care you want when you cannot make your own decisions. This plan is called an advance directive.
Gundersen is an international leader in advance care planning and has a wide range of resources that can help. We also have resources in the Boscobel area that can provide additional support.
To connect with an ACP facilitator, please call (608) 375-4112.
