What to expect

First 3-5 Days

  • Home Hospital care begins once you leave the Hospital
  • Your doctor works with a nurse who will care for you at home twice a day
  • A Recovery Care Coordinator is your main point of contact and available 24/7
  • All necessary equipment is set up at your home
  • A tablet lets you connect with your doctor and your care team

Your Registered Nurse

  • Takes your vitals, temperature and weight
  • Monitors IVs
  • Helps you take your medications
  • Assists you with pain management

Your Recovery Care Coordinator:

  • Available 24/7
  • Works with you and your doctor to arrange care at your home and coordinates with specialists
  • Tracks your vitals daily
  • Schedules follow-up appointments
  • Provides health education

Last 25-27 Days

After the first few days your care transitions back to your primary care provider. You will then resume your normal activities, but still have access to your Recovery Care Coordinator. They will be available 24/7 to answer questions, schedule follow-up visits and continue to track your progress.
