Accessing Records

Children under 12

As a parent, you can access your children's health record through MyCare until the child's 12th birthday.

Through proxy access, you can:

  • Connect with your child's doctor or care team
  • Manage immunizations and allergies
  • View test results
  • Manage prescriptions
  • Schedule appointments
  • Order contact lenses and more

Children 12-17

For children ages 12-17, this proxy form must be printed, completed, signed and mailed, or scanned and emailed for you to have access to your child's MyCare account.

If mailing, the form should be sent to: Gundersen MyCare, 1900 South Ave., AVS-001, La Crosse, WI 54601.

If scanning and emailing, the form should be sent to: mycare@gundersenhealth.org

Once a child turns 18, they are considered an adult and their account becomes private.

Adult Dependent

If you are a caregiver or guardian for an adult, and would like to manage their care online through MyCare, an adult proxy authorization form must be filed.

The form must be printed, completed, signed and mailed, or scanned and emailed.

If mailing, the form should be sent to: Gundersen MyCare, 1900 South Ave., AVS-001, La Crosse, WI 54601.

If scanning and emailing, the form should be sent to: mycare@gundersenhealth.org
