Bringing hospital care into your home

Gundersen Home Hospital care usually lasts between three and five days. Your trusted team of doctors, nurses and other specialists will monitor you daily using both virtual and in-person visits.

  • Tracks your vitals daily
  • Works with you and your doctor to arrange care at your home, and coordinates with specialists
  • Schedules follow-up appointments

What to expect: first 3-5 days

  • Care begins once you leave the hospital

  • All necessary equipment is set up at your home

  • Your doctor works with a nurse who will care for you at home twice a day

  • A recovery care coordinator is your main point of contact and available 24/7

  • A tablet lets you connect with your doctor and care team

What to expect: last 25-27 days

  • Care transitions back to your primary care provider
  • Resume your normal activities
  • You maintain 24/7 access to your recovery care coordinator for questions, follow-up visits and real-time recovery tracking
