At Gundersen, we participate in The DAISY Award® program, a nationwide program that rewards excellence in nursing. If you’ve been cared for by a remarkable nurse, this is your opportunity to honor them for the difference they’ve made in your life.
Honoring extraordinary nurses
The DAISY (Diseases Attacking the Immune Systems) Award recognizes nurses for their dedication to patients and families. It honors the legacy of Patrick Barnes, who passed away from an autoimmune disorder known as ITP.
The DAISY Award is the first of its kind to give patients, families and colleagues a way to express their gratitude for the compassionate care that nurses provide.

How to nominate a nurse
Who can be nominated?
You can nominate any Gundersen nurse practitioner, registered nurse or licensed registered nurse in good standing.
How can I nominate a nurse?
- Fill out our online DAISY nomination form (found below on this webpage).
- Complete a nomination form at one of our locations.
- Download, print and return a paper nomination form by mail.
How are winners selected and recognized?
Our DAISY selection committees review nomination ballots using a scoring rubric.
- Nominees receive a pin and certificate, while the honoree is also gifted with a hand-crafted statue.
- Celebrations occur annually and more frequently at some locations.
