Gundersen Health Accountable Care Organization (ACO), LLC was established in 2022.
This ACO is a patient-centered group of hospitals and clinics across Wisconsin, Minnesota, and Iowa that have joined to become members of a single organization where the patient and clinicians are partners in care. Members coordinate care for seamless, high-quality care for Medicare Fee-For-Service patients. Wholly owned by the participants, the infrastructure, governance, and Medicare Shared Savings Program (MSSP) requirements are centralized and managed by the members through the ACO Board of Managers. A standardized approach to care coordination, patient engagement, promotion of evidence-based medicine, and quality reporting has been implemented in each community.
Gundersen Health System’s mission is to provide quality care and great patient experience, improve community health, and reduce the cost of care. Gundersen Health ACO allows healthcare entities to carefully implement and evaluate new delivery models to understand their impact on the health, quality, and financial viability of their community and healthcare delivery system.
An accountable care organization (ACO) is a group of clinicians and other healthcare providers who voluntarily work together with Medicare to give you high-quality service and care at the right time in the right setting. Your clinician has agreed to participate in a Medicare Shared Savings ACO and to work closely with other clinicians and care teams to coordinate care. Utilizing data from Medicare, each ACO clinician will know your medical history, including your medical conditions, prescriptions, and visits to other clinicians, and be able to provide you with the right care at the right time in the right setting, while eliminating redundancies in care.
