Sports Medicine Physical Therapy Residency Application

Applicants for the Gundersen Sports Medicine Sports Physical Therapy Residency Program are required to have graduated from an accredited Master's or Doctoral entry-level physical therapy program. Possession or application for Wisconsin state physical therapy licensure is also required.

Candidates from entry-level physical therapy programs may apply for a temporary licensure to allow you to practice under the supervision of a certified PT while you await the passing of the board examination.

Along with completion of the application through RF-PTCAS, we request applicants complete this Gundersen Sports Medicine Physical Therapy Residency Application to alert us of your interest.

Applications will be accepted until December 1.
