Gundersen Health defines the Thriving Community Impact Fund as grants, in-kind gifts and sponsorships, and the fund supports programs and initiatives that align with our vision.
New to the Thriving Community Impact Fund
Our dedication extends beyond healthcare services. We understand that health begins within the community, and that vibrant, thriving communities are the foundation for healthy individuals. The Thriving Community Impact Fund is more than financial support—it is a catalyst for lasting change, rooted in collaboration and shared purpose.
Community Health Impact Programs & Activities
We welcome grant applications for programs and activities that enhance the health and well-being of our community members, closely aligned with the Health System’s Community Health Improvement Plan and Community Health Score.
Focus Areas:
- Physical, mental and social well-being
- Poverty and economic stability
- Improvement of neighborhood and built environments
There are now three applications to choose from:
Support Grants
Up to $2,499, a simple and quick application with a fast turnaround
- Open application period: rolling
- Processing period: 20th - end of month
- Funding decision: first of each month
Core Contribution Grants
$2,500 - $49,999, are for larger initiatives aligned with our community health priorities

Community Catalyst Grants
$50,000+, are for transformative projects with significant community impact
- Open application period, reviewed annually: September 1 - November 1
- Processing period: November 2 - November 30
- Funding decision: December 1
Event Sponsorship, Local Membership, and Community Engagement Requests
This is a new application that supports events that align with our vision, including health fairs, educational workshops, awareness campaigns, fundraising events and other local causes that promote community well-being.
We recognize the value of events and sponsorships for many purposes; however, whenever there is an opportunity to support programming that directly benefits individuals, that is our preferred focus for funding.
To ensure we have ample time for thorough planning and coordination, please submit your applications at least 12 weeks before your event date. Applications received less than 12 weeks in advance will unfortunately be declined.
In addition to event sponsorships, we also consider requests for local membership opportunities that support economic development within our communities. These memberships help foster partnerships, collaboration, and growth that contribute to the overall health and vitality of the local economy, which in turn supports community well-being.
Applications are accepted on a rolling basis.

Thriving Community Impact Fund Guidelines
To be eligible for funding, organizations must be verified as a non-profit, school, or government agency. With limited funds available, we are excited to prioritize requests that truly make a difference by:
- Addressing the vital needs highlighted in the Community Health Needs Assessment
- Championing health and wellness initiatives
- Driving impactful civic, community, and economic development
Guidelines for this program do not allow funding for the following:
- Political activities
- Requests from individuals
- Medical expenses for individuals
- For-profit entities
- Trips/travel/teams
