Community & Preventive Care Services

Gundersen's Department of Community and Preventive Care Services is committed to enhancing health and quality of life through a population health focus - impacting our patients, our workforce, and our communities. Our innovative wellness programming, community collaboration and partnerships, and corporate support and commitment are integral components that seek to address identified needs of the individuals and communities we serve. The most recent community health needs assessment outlines priorities in a document called COMPASS. An implementation plan developed by Gundersen addresses targeted areas. If you have questions regarding the community health needs assessment or implementation plan or would like to provide feedback, please contact Sarah Havens, phone 608-775-6580 or 1-800-362-9567, ext. 56580 or email
