As a recipient of Federal financial assistance, Tri-County Memorial Hospital Inc. d/b/a Gundersen Tri-County Hospital and Clinics, which includes clinics at Blair, Independence and Whitehall, Wisconsin; Gundersen Tri-County Care Center, and Gundersen Tri-County Senior Living, herein after "Gundersen Tri-County" does not exclude, deny benefits to, or otherwise discriminate against any person on the basis of race, age, color, national origin, disability, in admission to, participation in, or receipt of the services and benefits under any of its programs and activities, whether carried out by Gundersen Tri-County or through a contractor or any other entity which Gundersen Tri-County arranges to carry out its program and activities.

This statement is in accordance with the provisions of Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973, the Age Discrimination Act of 1975, and the Regulations of the US Department of Health and Human Services Issued pursuant to these statues at Title 45 Code of Federal Regulations Parts 80, 84, and 91. Other agency guidelines prohibit discrimination on the basis of creed, ancestry, religion, culture, language, socioeconomic status, marital status, newborn status, sex, sexual orientation, gender identity or expression, source of payment or any other status protected under applicable federal or state law.
