A great team: Joining together to combine our common interests and make a real difference in the aging of America.
As part of Respecting Choices' transition to C-TAC Innovations from Gundersen Health System, Respecting Choices will have new contact information effective January 27, 2017.
C-TAC Innovations is a self-sustaining nonprofit affiliate of C-TAC. The mission of C-TAC Innovations is to provide assistance to health organizations and other groups to develop person-centered models of care, especially for those with advanced illness.
The continued success of Respecting Choices required a larger, national platform for purposes of growth, improvement, and sustainability.
"We are thrilled to be part of C-TAC Innovations and expand our reach," says Bud Hammes, PhD, Respecting Choices executive director. "Most Americans will face advanced illness at some point—their own or a loved one's. And, the vast majority believes patients and healthcare providers should talk about advanced illness care issues. Yet, our systems today tend not to support coordinated, person-centered care and conversations like these. We are changing that."
