Thank you for your commitment to Respecting Choices. You are providing an invaluable service to your colleagues and your patients by offering Respecting Choices advance care planning Facilitator training.

Purchase online seats and submit information at least 6-8 weeks prior to your course date to allow sufficient time for your course participants to complete the online modules.

Submit a course request form

  • Instructor course request: For First Steps, Next Steps, or Last Steps ACP Facilitator Certification courses
    PLEASE NOTE: In order to teach a Last Steps Facilitator Certification Course after May 1, 2016, you must view the WebEx video presentation and purchase updated materials. To view, click here and enter password LS2016.
  • Faculty course request: For First Steps or Last Steps ACP Instructor Certification courses

PLEASE NOTE: If you are holding a Facilitator Certification Course in conjunction with an Instructor Certification, it is included in the Faculty course request form. If it is a Facilitator Certification course only, please use the Instructor course request form.

A Course Request Form is required for every course you teach. This is how your course gets into the system so you can get credit for teaching it, and also so we can connect your participants with the correct course.

Return sign-in sheet

Within 2 weeks after your course, email a copy of your sign-in sheet to info@respectingchoices.org. This is important so your participants are listed as certified Facilitators in our database and so you receive full credit for teaching the course.
