Seminar Planning Information

Gundersen Medical Foundation is accredited to provide Category 1 (CME) credit by the Wisconsin Medical Society.

If you are interested in hosting a seminar that is sponsored by Gundersen Medical Foundation, you must complete an application at least six months prior to your seminar. Below you will find the application link, as well as tools that will assist you in completing the application (Needs Assessment Information, Evaluation Form Information, Learning Objective Guidelines, Commercial Support Information, Faculty Disclosures Form and Honoraria Policy).

After completion, please submit the application to Jackie Ross, Gundersen Health System, 1836 South Ave., C03-006A, La Crosse, WI 54601.
