You need to heal but don't need a nursing home
Our goal with Swing Bed is to help you live the most active and healthy life you can, as quickly as you're able. In short, we want to get you "back in the swing" of life.
"Gundersen is dedicated to providing high-quality, compassionate care with a personal touch for the members of our community," says Heidi Cornell, Gundersen St. Joseph's director of Social Services.
"Our Swing Bed program is designed to promote the health and wellness of our patients by offering them a safe option where they are able to receive the skilled rehabilitative care they need close to home."
Swing Bed is the name of a Medicare-eligible covered, in-hospital program for patients who no longer need hospitalization, but do need rehabilitation or skilled nursing services.
Swing Bed could be for you if you are:
- Recovering from major surgery or an extensive procedure
- Strength training and rehabilitating after an injury or hospital stay
- Seeking coaching and education on living with a medical condition, such as diabetes
Swing Bed services include:
Skilled nursing care during your stay to assist with pain management, education and recovery.
Nutrition services to support, educate and customize diets and menus.
Physical therapy to help you work through injuries, build strength, prevent falls and more.
Occupational/recreational therapy to help restore daily living skills and independence, including bathing, dressing and basic housekeeping, and get you back to the activities you love. The safety of your home can be evaluated, as well.
Speech therapy to help with communication skills, reading and safe swallowing.
Pharmacy staff to work with your doctors to meet medication needs.
Social services and spiritual care to help you reach or maintain physical, mental and social well-being, including emotional and spiritual support to promote healing.
