Helping chronically ill patients access transitional services

If you have patients with chronic illnesses, then you know the transitional care they may require can vary as greatly as their conditions. To help you find the best options for your patients, Gundersen Health System has developed a new, comprehensive guide called Transitional Services for the Chronically Ill.

The document is a useful tool not only for evaluating transitional services, but also in introducing the sometimes-sensitive topics to your patients and educating them about available opportunities.

Transitional services in the guide include:

  • Advance care planning
  • Palliative care
  • Hospice care
  • Spiritual care
  • Care coordination

For each of these services, you'll find conversation starters to help guide discussion about the programs with your patients, as well as information about who is eligible, how to refer patients and costs, if any, associated with the services.
