Sports Physicals

Sports physicals are offered on Mondays at the La Crosse location and Tuesday's at the Onalaska location, August 5th through September, 2013. No appointment necessary.

Services can be billed to your insurance company and no copay is necessary at the time of your visit.

Important: Please bring the following items to your sports physical visit:

• Current immunization record
• If under age 18, parent must be present or a written permission with parent signature must be presented.
• Insurance information
• Completed and signed Sports Physical Form:
• Wisconsin
• Minnesota (print the "Sports Qualifying Physical Examination Clearance Form")
